It is often said as though it were a personal quirk, or worse, a bit of imagination.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is neither. Failure of <strong>dental anaesthesia<\/strong> is a recognised clinical problem with well described causes, and the most common of them has nothing to do with your nerves being unusual or your mind playing tricks. It has to do with chemistry.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Tooth That Hurts Is the Tooth That Resists<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The cruel irony of dental treatment is that the tooth most likely to resist anaesthesia is the tooth in most trouble. To cross that membrane, the drug has to be in its uncharged form, and whether it is uncharged depends on the acidity of the tissue it is sitting in. Infected and acutely inflamed tissue is more acidic than healthy tissue. In that environment a greater proportion of the drug stays in its charged form, which cannot pass through the membrane, so less of it reaches the place it needs to act.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><em>The Nerve Is Sensitised as Well<\/em><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">There is a second problem layered on top. Nerves in an inflamed pulp become sensitised, with changes to the sodium channels that anaesthetics target. The nerve is both harder to reach and harder to silence once you reach it. This combination is the honest answer to why won&#8217;t my tooth go numb when everything else in the mouth clearly has.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><em>Numb Lip, Wide Awake Tooth<\/em><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Put together, these explain something patients find baffling. The dentist gives an injection, your lip and tongue go completely numb, and the tooth still jumps when work begins. Soft tissue numbness and pulpal numbness are not the same thing, and successful dental anaesthesia of the gum does not guarantee it of the nerve inside the tooth.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Lower Back Teeth Are the Hardest<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Not all injections are equal. Numbing an upper tooth is usually straightforward, because the bone overlying the roots is relatively porous and local anaesthetic placed beside the tooth can diffuse through it.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Why the Lower Jaw Needs a Block<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The lower jaw is denser, so the same approach does not work. Instead the dentist has to deposit the solution near the nerve trunk before it enters the bone, which is what an <strong>inferior alveolar nerve block<\/strong> is. The target is small, it cannot be seen, and its exact position varies from person to person.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Studies of teeth with <strong>irreversible pulpitis<\/strong> have repeatedly found that an inferior alveolar nerve block on its own fails to produce profound pulpal anaesthesia in a substantial proportion of cases. That figure surprises people, and it should reassure them too. If a block did not work for you, you are in ordinary company, not a rare one, and irreversible pulpitis is exactly the diagnosis in which this is expected rather than surprising.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><em>Two Teeth, One Appointment, Different Results<\/em><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">It also explains why the answer to why won&#8217;t my tooth go numb changes from tooth to tooth in the same appointment. Accessory nerve supply, an unusually positioned nerve, and dense bone all shift the odds. So does anxiety, though not in the way it is usually implied. Being frightened does not neutralise the drug. It lowers your pain threshold and makes pressure and vibration much harder to tolerate, which can feel identical to the anaesthetic having failed.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What a Dentist Can Do Next, and What You Can Ask For<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The important message is that a failed first injection is a starting point, not a dead end. There is a sequence of options, and knowing they exist changes the conversation.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Supplementary Injections<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Supplementary techniques are the usual next step. An <strong>intraligamentary injection<\/strong> places the solution directly into the ligament space around the tooth. An <strong>intraosseous injection<\/strong> delivers it into the bone beside the root. Both bypass the problem of getting a drug to diffuse through inflamed or dense tissue, and both are ordinary parts of practice rather than exotic measures.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An intraligamentary injection is quick and needs very little solution, which is why it is often tried first. An intraosseous injection takes slightly more preparation but tends to be the more dependable rescue when a lower molar with irreversible pulpitis will not settle. Neither is a sign that anything has gone wrong.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Changing the Drug or the Technique<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Changing the drug or the approach helps too. <strong>Articaine<\/strong> infiltration alongside a block behaves differently from lidocaine alone in the lower jaw, and many dentists reach for articaine specifically when a standard inferior alveolar nerve block has under-performed. Buffering the solution to raise its pH addresses the acidity problem at its source. Alternative block techniques exist for cases where the usual approach keeps missing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is reasonable to ask for either by name. Saying that a previous intraligamentary injection worked, or that an intraosseous injection was needed last time, saves the person treating you a great deal of guesswork.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><em>Treating the Infection First<\/em><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Sometimes the right answer is to treat the infection first. A tooth with an acute abscess may become far easier to anaesthetise once pressure has been relieved and inflammation has begun to settle, which is one reason a dentist may prefer to drain, prescribe and rebook rather than push on. If the tooth needs <a href=\"https:\/\/www.aliadent.com\/en\/surgical-procedures\/root-canal-treatment-in-turkey\/\">root canal treatment<\/a>, that plan is common and sensible rather than a delay for its own sake.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><em>A Word That Should Have Disappeared<\/em><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">One term has been left behind, and it is worth naming. If a clinic still calls the injection Novocain, that drug has not been in routine dental use for decades. Say whether the numbness took hold and then faded early, or never really arrived. Agree a signal, usually a raised hand, and use it early instead of enduring the first few seconds to be polite. Ask, plainly, whether a supplementary injection is available if the first one is not enough.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If dental pain has been going on for some time, the NHS guidance on <a href=\"https:\/\/www.nhs.uk\/tests-and-treatments\/root-canal-treatment\/\" target=\"_blank\" rel=\"noopener\">what the procedure actually involves<\/a> gives a clear picture of the steps and of why leaving an infected tooth rarely improves matters.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The reassurance worth carrying out of this is simple. A tooth that will not go numb is a chemical and anatomical problem, not a character flaw and not something you invented. So the next time you find yourself thinking why won&#8217;t my tooth go numb, treat it as information to hand over rather than something to apologise for.
