Why New Spaces Appear After a Deep Clean

You went in with gums that bled when you brushed. You came out with small dark triangles near the gum line that were not in the mirror last week, along with teeth that suddenly look longer. It is an unsettling result to get from a treatment you were told was good for you, and the first thought most people have is that the hygienist took something away that should have stayed.

That is not what happened. But the reassurance usually offered, that the tartar was holding the space and the gums have shrunk as swelling went down, is only half an answer. It explains where the spaces came from. It does not answer the question you actually want answered, which is whether they are going to close again.

Where the Spaces Were Hiding

Calculus does not sit neatly on the surface of a tooth. It builds up between teeth and under the gum margin, filling the space where healthy gum tissue would normally sit. At the same time, inflamed gum is swollen gum. Chronic gingivitis makes tissue puffy, red and enlarged, so it drapes over the tooth and covers more of it than healthy tissue would.

Why They All Appear at Once

Remove the deposits and treat the inflammation and both of those disappear together. The tissue firms up and reduces to its true contour, and the space it was never really filling becomes visible. This is why the gaps between teeth after deep cleaning feel so sudden. They did not form during the appointment. They were formed slowly over years of gum disease and were being masked by the very thing that caused them.

Longer Teeth, Unchanged Gum

The same mechanism explains why teeth look longer. What you are seeing is the actual level of your gum, uncovered rather than newly lowered. It is worth being precise here, because patients often describe it as gum recession caused by the appointment. The gum recession was already present. The appointment made it visible, and the same is true of the spaces themselves.

The Measurement That Predicts Whether the Triangle Fills

These spaces have a name in dentistry, black triangles, and this is the part almost nobody tells patients about them. It converts a shrug into a genuine prognosis.

Contact Point to Bone Crest

Whether the small wedge of gum between two teeth, the interdental papilla, can fill a space depends less on how well you brush than on a simple distance: how far it is from the contact point where the two teeth touch each other, down to the crest of the bone between them.

What the Measurements Found

A well known set of clinical measurements published in the Journal of Periodontology in 1992 found that this distance is strongly predictive. Where the contact point sat five millimetres or less above the bone crest, the interdental papilla filled the space almost every time. At six millimetres it filled roughly half the time. At seven millimetres it was present in only around a quarter of cases. Each additional millimetre roughly halves the chance.

The reason is structural. Gum tissue is supported from underneath by bone. If gum disease has destroyed the bone between two teeth, the papilla has nothing holding it up, and no amount of careful brushing, gum gel or patience will levitate it into position. That is why black triangles behave so differently from person to person despite identical home care.

Not a Complication of the Cleaning

It is worth saying plainly, because the internet rarely does: black triangles are not a complication of scaling and root planing. They are a consequence of the disease that made the treatment necessary.

The Question Worth Asking

This is a measurement your own dentist can take from an existing radiograph and a periodontal chart. It is a fair and specific thing to ask: how far is my contact point from the bone, and does that put me in the range where the interdental papilla is likely to come back?

What Can Reasonably Be Expected to Improve

Some genuine improvement usually does happen in the weeks after treatment, and it is worth knowing which parts.

The First Six to Eight Weeks

Tissue that was swollen keeps firming and maturing for several weeks, so the appearance at day three is not the final result. Bleeding should settle substantially. Pocket depths typically reduce as inflammation resolves and tissue tightens against the tooth, which is exactly why a re-evaluation appointment is normally scheduled six to eight weeks later rather than immediately. Those pocket depths, recorded again at that visit, are the number that tells you whether the treatment worked.

What Will Not Grow Back

What does not happen is the regrowth of lost attachment and bone in the ordinary course of healing. Non-surgical periodontal treatment, the course usually called scaling and root planing, is very effective at stabilising the disease. It does not rebuild what was already destroyed. Understanding that distinction early spares a lot of disappointment, and it reframes the goal correctly: the aim is to stop the loss, not to reverse it.

It also explains something that sounds like a contradiction. A successful course of scaling and root planing can leave you with slightly larger gaps between teeth after deep cleaning than you had before, precisely because it worked. Shrinking, healthy tissue sits lower than swollen, diseased tissue.

Closing the Space From the Other Side

If the spaces bother you, the honest fix works from the other direction. Rather than trying to move the gum up, a dentist moves the contact point down, by adding a small amount of composite to the adjacent surfaces so the teeth touch closer to the gum. In orthodontic cases, reshaping and closing the teeth slightly achieves the same thing. Both are aesthetic solutions applied after the disease is under control, never instead of controlling it, and neither is worth considering until pocket depths have stabilised.

The Part That Actually Decides the Outcome

There is one more thing the spaces change, and it matters more than how they look. The gaps between teeth after deep cleaning are not only a cosmetic question, because larger interdental spaces need different cleaning. Floss slides through a tight contact but does very little in a wide one, and this is the single most common reason people go back to bleeding gums a year after a course of treatment.

Choosing and Sizing the Brushes

Interdental brushes are the tool for this, sized so that each one enters with gentle resistance without the wire scraping enamel. Most people need more than one size for different parts of the mouth. Expect some bleeding for the first week or two while inflammation settles, and expect it to stop. Bleeding is the sign that tells you where the disease is, not evidence that the brush is causing harm.

Getting the sizes right matters more than brand. Ask the hygienist who carried out your scaling and root planing to size your interdental brushes for you at the review appointment, space by space, and write the sizes down.

Keeping It From Happening Again

Used properly, interdental brushes are also what stops further gum recession, because they remove the plaque sitting at the margin where recession advances. Regular professional cleaning keeps the deposits from re-forming in the spaces that are now open, and the interval matters more for someone with a treated periodontal history than for someone without one. So the gaps between teeth after deep cleaning are real, they are not damage, and whether they close is largely decided by a distance that can be measured rather than by luck. Ask for the measurement. If the answer is that the bone sits too far below the contact point, you at least stop waiting for something that was never going to happen, and you can decide about the cosmetic options with clear eyes. And if the answer is that you are in the range where the tissue can fill in, the best thing you can do is keep those spaces genuinely clean while it does.
